It’s caused by blockage of the ducts in sebaceous glands like the Meibomian glands. Typically painless, it may become painful if infected with bacteria.

Touching your eyes: frequent rubbing or touching can introduce bacteria and irritate the oil glands
Chronic skin conditions: rosacea or seborrheic dermatitis
Poor eyelid hygiene: not removing make-up thoroughly. This allow bacteria to build up on your eyelids and lashes
Blepharitis: inflammed eyelids
